For patrons who are community members and not affiliated with our partner organizations, please complete the
Special Borrower's Form in order to check out materials.
For Alumni and Special Borrower patrons, if you are unable to locate your book in the Stacks, please submit this Special Borrower’s Retrieval Form and a member of the Circulation department will conduct a search and pull your requested materials. Please allow up to 3 business days for staff to complete search and follow up with you
A proxy card allows another individual to check out items on your USF account. This can be especially helpful when you (the sponsor) are personally unable to visit the Library but where the other individual (the proxy) can obtain the item in your place. For instance, a USF faculty member may want to obtain a proxy card for their graduate assistant.
For patrons who have returned materials but still have lost books on their patron account, please complete this form
If you have been assessed a library fine, but you feel that you should not have been, please complete the Dispute a Fine Form.
Please allow 72 hours for staff to conduct a thorough shelf search and complete fine resolution process
Please use this form for items that are currently checked out to you and that you cannot find. You will be billed for the lost item.
If you are looking for an item that is not on the shelves, please complete the "Item Search Request Form" and we will look for that item.
For patrons experiencing issues renewing their materials, please complete the Online Renewal Form
Use this form to report any maintenance issue that you encounter. Submission will be routed to our front desk staff and facilities team.
USF Libraries depend on users to know the volume etiquette of the library and work together to create places that are conducive to reading and studying. The issue will be routed to on-duty staff and they will respond as staffing permits.
